The Arizona Department and Health website has information on the immunization requirements for all school age children.

To enter Kindergarten students must have the following immunizations:

4-5 doses of DTaP

3-4 doses of Polio

2 doses of MMR

3 doses of Hepatitis B

1-2 doses of Varicella, depending on age of child when vaccine was given.

When entering 6th grade, the state of Arizona requires:

1 dose of Tdap

1 dose of Meningococcal

Just a reminder to parents that Arizona state law (A.R.S. 15-344):

* Requires medication to be delivered to the health office by a responsible adult and not the student.

* Medication must be in the original container prepared by the pharmacist.

* A form will need to be completed by the parent giving permission for the school personnel to administer the medicine for your child.

* This includes prescription and over the counter medications.

If you have any questions or concerns, please contact the health office at the school. Thank you for your cooperation in this matter.
